These Terms & Conditions govern your use of our sign-up form for consideration to participate in our study involving a software-based medical device (“SaMD”). By submitting the sign-up form, you agree to these Terms.2. Eligibility
You must be 18 years of age or older.
You must be able to provide informed consent independently.
Providing false information will result in disqualification.3. Purpose of Sign-Up
Completing the sign-up form does not make you a participant in the study. It allows us to collect preliminary information to send you a consent form and reach out to you for marketing purposes.4. User Responsibilities
Provide accurate, truthful, and complete information.
Use our services only for lawful purposes.
Do not attempt to interfere with or misuse our systems.5. Data Use
Your personal data will be handled according to our Privacy Policy. By signing up, you agree that we may process your personal and health data for the purposes described there. You can ask for removal of your data by sending us an email to [email protected]6. Marketing Communications:
We will send you marketing communications, unless you specifically request us to remove your contact from our mailing list, by sending us a written request to [email protected]7. Liability
